- You will be contacted and sent to 1 of our 4 convenient locations for collection (or) lab kits can be mailed nationwide
- Results typical within 48-72 hrs after labs drawn
HEALTH HISTORY INTAKE
Prior to being evaluated, patients must complete a Health History Intake and Consent forms.
- Fill this out at the office when you come for your lab work (or)
- Download, print, fill out and either email or bring in completed form.
VISIT / CARE PLAN CONSULTATION
Once your Health History Intake forms and Lab Work has been received it will reviewed by one of our Health Care Providers and you will be contacted so an appointment can be made to discuss your Care Plan.
Rx DELIVERED TO YOUR DOOR
Depending on your Health History, Lab Work and Care Plan discussed, you may receive a prescription. Your prescription will be delivered to your home or picked up in office! Upon arrival you will be contacted by a patient care coordinator to ensure proper dosing and utilization. Our office and patient care coordinators can be reached during office hours to answer any and all questions you may have.
